I agree that I am the applicant whose information is set out on this document and that by the completion of this document and its return by electronic means I am: 1. making application for membership of the Construction, Forestry, Maritime, Mining and Energy Union (CFMMEU) and the Construction, Forestry, Mining and Energy, Industrial Union of Employees, Queensland (CFMEUQ) on, and from, the date this document is received by the District Secretary/Divisional Secretary or their delegate; and 2. agreeing to abide by the rules and policies of the CFMMEU and the CFMEUQ as amended from time to time.

You are required to pay the entrance fee, subscriptions, levies and fines in accordance with the Part C Rules of the CFMEUQ and the rules of the CFMMEU, Mining and Energy Division. Any member who fails to pay in accordance with these rules may become unfinancial and will not be entitled to any of the benefits and privileges of membership.

You may resign by written notice to the District Branch Secretary/District Secretary. Resignation will take effect on the day specified in your notice or, in any other case, two weeks after notice is received. Unless you otherwise specify, resignation from membership from the CFMMEU will be treated administratively as resignation of the CFMEUQ, and vice versa.

Resignation does not affect your liability as a member to pay any monies that were owing by you at the date of resignation, and such monies may be sued for and recovered in accordance with the rules of respectively the CFMMEU or CFMEUQ.

The CFMMEU and the CFMEUQ collect, hold, use and disclose personal information to carry out their functions and/or activities under the Privacy Act 1988 (Cth).
